The application form and recommendation form are available online in Adobe Reader/Acrobat (*pdf) or in Microsoft Word (*.doc) format. This form can be printed out or filled out online and mailed to us.

Potential students should complete the application form and work with their supervisor or HR to determine if the agency has an internal process for selecting candidates. Many state agencies have internal processes. If you are selected by your agency, please see that your completed application is sent to the CPM program at the address below. If you agency does not have an internal process, complete the application and recommendation form and send to:

Academic Credit:

CPM courses have the same transferability as other Willamette University MBA courses.

Students who successfully complete the CPM program who are admitted to Willamette University's Full-time MBA program, may transfer 12 credits of CPM course work to the elective portion of the MBA degree.

Students who successfully complete the CPM program who are admitted to Willamette University's MBA for Professionals program may transfer 8 credits of CPM course work to the MBA degree.
