Arriving for Fall Semester
School starts on August 18th! Everyone must be here and ready to jump into the career-building and life-changing experience of the Willamette MBA.

Immunization Records

Oregon state law requires all new students to provide immunization and health records. The University Health Center is moving to an online form with online submission. The new Health History form will go live after July 6th. You should complete and submit the form by August 10th.

To complete the form you will need to know the dates of your MMR immunizations (Measles, Mumps, Rubella), so be sure to know your the dates of your immunizations.

If you need the measles immunization (MMR), you can get it on campus during Compass Week. Charges for vaccinations received on-campus will be billed to your Willamette University student account.

[Note: If you have recently been enrolled in the Willamette University College of Law or College of Liberal Arts, you should update the health history form, but you do not need to resubmit the immunization and history forms.]
