This form is for student or faculty-initiated panels.  If you wish to apply individually, please see the individual/co-presenter form.

A copy of this application will be emailed to all presenters and your faculty sponsor. It is the student's responsibility to contact the faculty sponsor for support or guidance of the presentation.

Students may choose to moderate their own student created panels.  However, a designated faculty member will moderate each panel unless the student panel members request otherwise.
