Key Request Forms
To submit a request for new keys or for lost or stolen keys:
- Download the approprate form below.
- Word documents can be completed using a computer and then printed.
- PDF documents can be printed and filled out by hand.
- Obtain the required signature(s) and then submit the form to Craig Nordlien in the Facilities Management Office, either by campus mail or your may fax it. Fax #: (503) 375-5366
