Travel Physicals
Students planning to travel abroad are often required to complete a travel physical. This physical can be done at Bishop Wellness Center by our staff. We schedule these appointments in advance. There is a fee for this physical. Please check with the medical receptionist for the current fee.
Before your Appointment
- Check http://wwwnc.cdc.gov/travel for information on each country you will be visiting
- Check http://travel.state.gov/travel for country entry requirements (immunizations, diagnostic tests, etc)
- Locate your immunization history and bring it to your appointment
- Fill out this form- Pre-Travel Questionaire to the best of your knowledge and bring to your appointment
If you arrive for your appointment without completing these steps you will be rescheduled
