1. Who is Covered
All FT, continuing employees who work at least 30 hours per week (.75 FTE)

Income replacement at 60% of salary (mo. max of $6,000) beginning a the ninety (90) day mark of total disablity, and 100% replacement of the employer retirement contribution in case of disability.

Willamette University "self insures" this benefit from the 90th to the 180th day of total disability, after which our LTD carrier picks up the claim.
[back to top]4. Disability Defined
The inability of an employee to perform the material duties of his/her job because of an illness or injury. Rehabilitation or training for another occupation will be reviewed after a 2-year duration on disability.
[back to top]5. Partial Disability
If an employee is partially disabled during the first 90 days, and returns to work on a part-time basis after you have completed your elimination period, you are eligible for partial benefits.

If an employee is disabled prior to the attainment of age 60, benefits are payable up to age 65, but not less than 5 years (60 months). Disabilities which occur after the attainment of age 60, will be paid in accordance with the plan provisions in the Summary Plan booklet.
[back to top]7. Exclusions
If you have received medical treatment, consultation, care or service including diagnostic measures, or take prescription drugs or medicines in the 3 months (90 days) prior to your effective date, this benefit will not be payable for any disability that occurs due to that illness or injury during the first 12 months (1 year) of this policy.

Social Security and Worker's Compensation share in the 60% benefit.
[back to top]9. Survivor Benefit
This featur will pay a 3-month lump sum to your spouse or dependents if you die while in receipt of disabiltiy benefits and have been disabled at least 180 consecutive days.
[back to top]10. Cost of Living Adjustment
This feature provides inflation protection by increasing the benefit an additional 3% compounded annually for the duration of the claim. This benefit begins after receiving 12 months of benefit payments.
[back to top]11. Retirement Income Protection
If you have been participating in Willamette's 403(b) retirement plan for at least 3 months, this benefit will pay 10% of your monthly salary to your 403(b) retirement plan whil you are receiving monthly disability benefits.
[back to top]12. Mental Illness Limit
Mental or nervous disabilities will be limited to 24 months (2 years) unless the disability is severe enough that the claimant is confined in a hospital.
