Personnel Action Forms
The following forms are to be used for initiating the transactions described. These forms are currently being tested with the intention that they will replace the "one size fits all" Personnel Action Form previously in use. We also hope to improve administration by having all PAF's submitted via scan/email process to the PAF submission email below. Eventually, we hope to establish webforms for each to further simplify these processes. Thank you to those who have provided feedback to help us improve processes in HR! If you use one of these test forms, please provide us with any feedback you have as we refine this new process.
