Form Instructions

Except for the online forms, the ability to use any of the other three formats requires that your computer have the software in which the form was created (i.e. Microsoft Word, Microsoft Excel or Adobe Acrobat).

Online Forms- UNDER CONSTRUCTION- Please use Microsoft Word Form.

The Travel Request, Group Travel Request, and Traveler Profile can be completed online via a web browser. Once you have entered all requested information, click Submit and the form will be emailed to the Travel Center. (A copy will also be delivered to you via email for your records, and a printable version will appear on screen when the form has been successfully submitted.)

The online form option is not available for the Expense Report because the form must be approved by a budget officer prior to submission.

Microsoft Word Forms

Word Forms are specially prepared Word documents that allow you to fill in the form without accidentally changing the text or the layout of the form.

Fill in the form by entering information in each form field. Use the tab key, arrow keys, or the mouse to move between fields. (In a form, a field is a location where text is typed, a box is checked, or information is selected from a list). The amount of space in a field may be limited, so responses must be concise.
This form does not make the calculations automatically, so do the math carefully!
Save the document to a local disk.
Print the form when it is complete.
Some parts of the printed form may have to be filled in manually and signed or initialed, as indicated.

Microsoft Excel Forms

The spreadsheet is protected so that you can only type in the cells at the top and in the cells under Amount (except where calculations are made automatically).

Press the tab key or use the mouse to move to the next cell.
Enter dates, descriptions, and amounts.
Calculations will be made automatically in the Totals section.
Save the document to a local disk.
Print the form when it is complete.
Some parts of the printed form may have to be filled in manually and signed or initialed, as indicated.

Adobe Acrobat Forms

The Acrobat version of the form is for printing and filling out by hand only. Supply all requested information and return the form to the Travel Center.
